DLG Scholar Athlete Application Form

Welcome! Thank you for applying as a student athlete for the Davis Law Group Scholar Athlete Program. To be elibible for the Davis Law Group Scholar Athlete of the Week, you must have at least a 3.0 GPA, have enough credits to be recognized as a senior and perform as a major contributor in your sport(s). If you are an peer, parent, coach, teammate, teacher or administrator. Please fill out the Scholar Athlete Nomination Form.

A Scholar Athlete is selected each week. One male and one female Scholar Athlete selected from the weekly winners at an end-of-the-year banquet to be recognized as the Davis Law Group Scholar Athlete of the Year. If you don’t get selected this week, we will keep your application to be considered throughout the year.

We encourage you to ask a friend, family, teacher or coach to also nominate you for your scholastic and athletic performance! Thank you for your application!
